I am looking to set up a form with selectable mailto addresses. The choices would be listed as checkboxes and users could send the info in the fields to several people quickly.

Example:

Name (field)
Email (field)
Send to: Alex (option)
President Clinton (option)
Hustler Magazine (option)
MTV (option)

The user could select any/all of the recipients.
