Looks like you have not correctly configured mysql.cfg.
Also try adding use lib '/full/path/to/cgi-bin/mysql'; at the top of the mysql.cgi as mentioned quite a few times in this forum.

There also looks to be a trailing slash where there shouldn't be.

Were you able to get this fixed?
I am having the same problem and have verified all of the paths.
There is a reference in the readme file for GT_Base.pl
and GT_Template.
These files were not in the .zip file.

I think that is because the directory structure has changed in MySQLMan now. The modules are kept in /Gt/. I expect Alex and Co. just havn't had time to update the readme
